Job Description:
A company is looking for a Support Broker (Bilingual) to assist
individuals with disabilities or chronic illnesses in managing
their long-term home care.
Key Responsibilities
Manage a workload of 110-145 program participants, maintaining
contact and resolving inquiries
Provide practical skills training in areas such as recruiting,
budgeting, and community resource identification
Assist participants with enrollment, developing spending plans, and
monitoring health and safety
Required Qualifications, Training, and Education
Minimum of two years of experience working with individuals with
disabilities and/or senior citizens
BA or BS degree in a related field, or 4+ years of related
experience in lieu of a degree
Bilingual in Spanish
Knowledge of Medicaid services preferred
